Eight tiers, and these are the actual drop rates. An Ultra Mythic turns up once in a hundred thousand scans, so most players will never see one.

Any standard product barcode — EAN, UPC, ISBN on a book, the label on a parcel. If your camera can read it, the game can turn it into a creature.
Yes. The code is hashed to build the creature, so a given product always produces the same one, anywhere in the world. That is what makes trading and comparing collections worth doing.
No. Scanning, hatching, collecting and battling all work offline. You only need a connection to share finds, use the league table, or battle someone over Wi-Fi.
Every hatch has a small chance of producing a shiny version, with a different palette and a sparkle effect. They are cosmetic, but they are the thing collectors chase.
